Glide The MOUD Case Manager I connects clients with opioid use disorder to medication-assisted treatments and Glide Foundation resources to support stabilization. This role manages a caseload of 20 to 30 participants, focusing on treatment adherence and access to housing, mental health, and other client needs.
Responsibilities
- Ensure clients have knowledge and access to Glide Foundation resources to achieve and maintain stabilization
- Work with participants to connect them to opiate replacement therapies such as methadone, suboxone, and naltrexone
- Create and develop a caseload of 20 to 30 participants
- Maintain treatment adherence and stabilization for participants
- Emphasize assistance with housing resources, identification, and mental health access
- Provide intervention and counseling to support positive health outcomes
