Responsibilities

Perform comprehensive physical therapy evaluations in patients' homes
Develop individualized treatment plans based on physician orders and patient needs
Provide skilled therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence
Assess and monitor patient progress toward established goals
Educate patients and caregivers on treatment plans, home exercise programs, injury prevention, and safety techniques
Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate patient care
Recommend and train patients on the use of assistive devices and adaptive equipment
Complete required documentation accurately and within established timeframes
Participate in case conferences and interdisciplinary team meetings
Identify changes in patient condition and communicate concerns to the appropriate healthcare providers
Ensure compliance with agency policies, Medicare regulations, and professional standards of practice
Promote patient independence and maximize functional outcomes

Required

Current and active Texas Physical Therapist (PT) License
Graduate of an accredited Physical Therapy program
Minimum of one (1) year of Physical Therapy experience
Valid driver's license, reliable transportation, and proof of automobile insurance
Current CPR certification or ability to obtain upon hire
Strong knowledge of physical therapy assessments, treatment planning, and rehabilitation techniques
Excellent communication, interpersonal, and patient education skills
Ability to work independently and effectively manage a flexible caseload
Ability to maintain accurate and timely electronic documentation

Preferred

Two or more years of Home Health Physical Therapy experience
Experience working with geriatric and medically complex patients
Familiarity with Medicare Home Health regulations and documentation requirements
Experience using WellSky (Kinnser) or similar electronic medical record systems
Knowledge of OASIS documentation
Experience developing and implementing home exercise programs
Bilingual (English/Spanish) preferred
Doctor of Physical Therapy (DPT) degree
