• What You Will Work On:
• Lead psychosocial rehabilitation and life-skills groups, teaching independent living and illness self-management skills
• Support members' community integration and vocational readiness, connecting them to opportunities and resources that promote long-term wellness
• Partner with psychiatrists, therapists, primary care providers, and peer support in treatment planning and coordinating member care
• Partner with referring provider to provide updates on member’s progress
• Support in driving member engagement – outreach to unengaged members to reintegrate them with care plan,
• Connect members with community resources and supports that promote wellness
• What You'll Have:
• Bachelor’s degree in a behavioral health or related field (e.g., Psychology, Social Work, Behavioral Sciences, Public Health, Sociology, Human Services, etc)
• 4+ years of behavioral health experience, or equivalent experience consistent with California’s MHRS qualification pathways under Title 9
• Up to two years of graduate professional education may count toward the experience requirement, consistent with California MHRS qualification pathways
