Distro The Physical Therapist Assistant (PTA) provides therapeutic services under supervision to improve patient mobility and function in a home health setting. This role involves implementing treatment plans, documenting progress, and educating patients and caregivers.

Responsibilities

Provide physical therapy treatments as prescribed by supervising Physical Therapist and physician
Implement individualized treatment plans to improve mobility, strength, balance, coordination, and functional independence
Observe, document, and report patient responses and progress
Assist patients with therapeutic exercises, gait training, transfer training, and mobility activities
Educate patients and caregivers on home exercise programs, safety techniques, and injury prevention
Communicate effectively with Physical Therapists regarding patient progress and treatment outcomes
Maintain accurate and timely clinical documentation in electronic medical record system
Monitor patients for changes in condition and report concerns
Participate in interdisciplinary care planning and case conferences
Ensure compliance with Medicare regulations, agency policies, and professional practice standards
Promote patient safety, dignity, and independence
Manage assigned patient visits efficiently

Qualification

Reliable transportationPrevious Home Health experienceExperience working with geriatricBilingual (English/Spanish)

Required

Current and active Texas Physical Therapist Assistant (PTA) License
Graduate of an accredited Physical Therapist Assistant program
Minimum of one (1) year of Physical Therapist Assistant experience
Strong knowledge of physical therapy treatments and rehabilitation techniques
Excellent communication and interpersonal skills
Ability to work independently in a home health setting
Reliable transportation, valid driver's license, and proof of automobile insurance
Current CPR certification or ability to obtain upon hire
Strong documentation and organizational skills
Ability to maintain patient confidentiality and comply with HIPAA requirements

Preferred

Previous Home Health experience
Experience working with geriatric, orthopedic, neurological, or medically complex patients
Familiarity with Medicare Home Health regulations and documentation requirements
Experience with EMR systems such as WellSky (Kinnser) or similar platforms
Knowledge of OASIS-related home health processes
Bilingual (English/Spanish)
