Salary: $70,000 annually

Position summary: Provide compassionate, coordinated medical, social, and support services to help older adults remain safely and independently in their homes as part of an interdisciplinary care team.

Coordinate community resources and referrals

Required

Strong documentation and EHR skills
Ability to work within an Interdisciplinary Team (IDT)
Knowledge of community resources and referrals
Excellent communication skills
Valid driver’s license and ability to travel locally for participant care
Ability to maintain confidentiality and professional boundaries
Willingness to obtain BLS certification if not currently certified
Preferred experience (Nice-to-Haves):
2+ years of healthcare social work experience
Experience with older adults or geriatric populations
PACE program experience
